one of my small fish just kinda swims with its head towards the bottom and the tail is just wanting to go upwards. if he has nothing above him he just points like one of those toys that just stays on its head. kinda line and arrow pointing down. he still eats blood worms and i am treating him w/kenaplex for internal illness and salt w/ temp at 86. will this work???

It's worth a try but I never had much success with this problem.
One thing I can recommend is for you to lower the water level as much as you can to help him heal.
You also might want to dose 1 teaspoon of epsom salt per 10 gal to clean out the content of his stomach.
He could be constipated and have gas.
Sorry that's all I have to help you bro.

Any grocery store will have them around the drugs bandaid area.
Do remember that epsom salt is not a medicine, it only acts as a laxative to make your fish empty his stomach content.
1 dose will do the job within 24 hours. So after 24hr and you see no changes, it would mean the problem is not constipation and gas.
I would use this method first before I use any medications; just a process of elimination.
Sometimes we overlook minor things and get too excited with medications and innocently kill the fish.
Good luck.
